NTSB says second mate asleep at the wheel before Nathan E Stewart disaster

The U.S. National Transportation Safety Board has released its report on the grounding of an articulated tug and barge northwest of Bella Bella last October.
The NTSB says the likely cause of the incident was the second mate of the Nathan E Stewart falling asleep while on watch.
The investigation revealed that the second mate missed a critical course change which resulted in the tug’s grounding in Seaforth Channel.
The probe also revealed that while the tug’s electronic chart system had an cross-track error alarm which would alert the operator if the vessel’s GPS position deviated from course, it was not being used at the time.
The grounding of the Nathan E Stewart fouled Heiltsuk waters with approximaelty 110,000 litres of diesel and other contaminants.
